Indian visa - one or two queries

I need to complete a visa application form for a holiday to Goa in November. As it's a holiday do I need to complete the 'Business Address' section? Also, although I have a long term partner I'm not married. Should I put his details in the 'Spouse' bit? And Finally, when they ask for my mother's name do they mean her married name or her maiden name? Thanks very much, Jay.

1. Is there also a question for employer's address? If not, provide that address in the "Business Address" section.

2. Don't give your partner's details in the "Spouse" section - you'll just confuse the issue!

3. Give your mother's single surname - presumably they want it for some sort of check.
