SD stands for Secure Digital. Bigger ones like these are used in digital cameras.
The mini ones used in mobile phones are like these.

An even more basic answer is that phones have a certain amount of memory built in that you can use to save phone numbers, photos & mp3's etc.
With some you can slot in an additional memory card (SD is a type of memory card) so that you can add much much more capacity to save all your mp3's and pictures & videos to
