Questionnaire Communication and networking.Help!!

Hello i have been asked to do a Questionnaire at work on how we communicate and network within are selves as a buisness. 3 main questions are

How well are we doing this?
What can we do to make things better?
What we are doing wrong?

i need to get this done very quickly but can't get a good short set of questions.
Could anyone please help?

A possible supplementary question is whether spelling words correctly (like 'ourselves' and 'business') aids communication

Anybody else help me on this without the obvious 3 questions (elaborate more)???

1. Describe the methods of communication you most commonly use with co-workers?
2. Explain the most common ways your co-workres communicate with you
3. Identify the poblems/concerns you have with the above utilized methods.
4, Describe the ways you feel would most effectively improve inter-office cmmunication.

Teachr tip: uing words like what will get you one word answers. Words like Identify, explain will make them think. :)
