Intravenous sedation is THE answer. Usual anaesthetics stopped working on me about 10 year's ago, so I was forced to find a dental practice which gave IS. Once you have it, all fear (and pain) vanishes. After treatment you're up and out of there very quickly, with no unpleasant after-effects. I paid �90 for my last sedation, but work undertaken was on the NHS. Considering you hopefully only have a check-up every 6 months, I think any resulting treatment is well worth paying that amount for. You don't even feel the needle go in your arm, so for me, there's nothing better.
