According to my book of cocktails the highball was created by New York bartender Patrick Gavin Duffy in the 1890's consisted of 2 ingredients (one spirit & one mixer). As it was such a potent drink it got it's name from railroad signal talk(!) a raised ball on a pole was a sign for the engineer to speed up (presumably the alcohol works quickly in highballs).
