Usually in your local Yellow Pages you can look under "Mental Health" and find some listings.  It's a place to start, anyway.  Is there a separate listing in your phone book for local government offices?  You might also check there to see what is listed under "social services."

Just ring up your local Social Services & request an assessment of your needs. They will then come to your house & assess whatever it is that you need help with. I'm pretty sure that's how it works.

Look up your local council web site and it should give you contact details for social work department.  You want you nearest area team office and you should phone and speak to the duty worker.
