I purchased an item from a website in America; the product value was around 32 + 1.50 for conversion charges. My account seems to be down approx 67. This would lead me to believe that I have been charged three times for the item (overcharged twice). At the moment the transactions haven't appeared on my online statement, only what my available balance is. I should be able to tell in the next few days when the transactions appear on my statement. . If this has happened to me what actions should I take? Bearing in mind this is a visa debit card.

Your bank will charge for the conversion too don't forget - most likely they wil anyway.
Did you pay direct from your BA?
Was the conversion you paid for the web sites charge?

The conversion was charged from the bank. I have no problem with that. It is standard procedure when you purchase items in a foreign currency, they convert them at the current exchange rate and ad their charge on top, in my case their charge was 1.50.
Was there a delivery charge that you possibly overlooked?
