I'm looking at hiring a car in Spain this August. I drive an automatic because I have arthritis in my left ankle and it makes it much easier to use my right foot on the pedals.

Does a left hand drive automatic car have pedals the same as right hand drive ie acclerator on the right and brake to the left, so I would still use my right foot.

YES! - and it`s not just automatics. Can you imagine all those British drivers having to cope a different pedal arrangement as well as driving on other side?
