Can you help to find this destroyer?

I've searched loads and loads of web sites trying to identify a WW2 destroyer number D45. It might seem simple enough but I've had no luck in finding it on all the naval sites. Can anyone help please? TIA.

Be careful - RN ships rarely change their names after being commissioned but their pennant number is subject to reallocation when they move to a new command. Pennant D45 is also recorded as being allocated to HMS Westminster and HMS Teaser.
