What document shows for definite whether a property is held as joint tenants or tenants in common?

You need to look at the Registered Title of the property, available for a small fee from the Land Registry. You can download it online.

If the property is not registered, it will be on the Title Deeds
The proprietorship entry will show the names of the people owning the property, but if held as tenants in common it will also say something like -
"No disposition by a sole proprietor of the registered estate (except a trust corporation) under which capital money arises is to be registered unless authorised by an order of the court".
