What is the significance of the 'And' credit in the cast list for films and TV?

The positioning of an actors name in film credits, advertising, billboards etc is very important to actors as it shows their precedence, and their position will be part of the negotiations as important as their pay.

 

So there are various positions, above the title, after, preceded with 'with' or 'and', 'special guest' - John Smith in 'Another Lousy Movie', starring, Joanne Soap, Brent Gay, with Sam Smith, J R Other, special guest star, Mortimer Wheeler. all very important to the actors, as is the size of font used, positioning etc etc etc.
So, as a follow up to that answer, what is the significance of "as" in the titles e.g. Joe Blogss as "Poochie"?
