What's the meaning of "tinkles" and "tune-up" in the following sentences?

"I'm afraid your arrival has taken us a bit off guard, sir."
"I know. I'm off my schedule, I am. Between you and me, the new medication gives me the tinkles. Thought I'd come over for a tune-up."

"Tinkle" is a polite word for "urinate" and is the sound made when someone urinates. When the man says "gives me the tinkles" it means the medication is making him urinate more often.

A "tune-up" means making adjustments to something to improve performance (usally an engine). In the example, the person's medication is making him urinate too often so he wants the doctor to change the medication and make things better for him.
