pregnant travel

My wife is currently 5 months pregnant. We are considering a holiday when she would be 30 weeks on departure, 32 weeks on our return. Options are Spain, Portugal or Florida. Does any one share our situation &/or know of any immigration rules for entry into the US. We understand a doctor's letter should satisfy most airlines.
