Personalised Reg Plate

I want to get a personalised reg plate for a birthday present. What fees do I need to pay apart from the price of the plate (duh!), retention fee, VAT and transfer fee? What is retention and transfer fee? Do I buy the plate and then get it registered? Any advice would be great.
