It seems that the length of the question didn't allow room for the reply button! Anyway, my reply is:
Really, really tough one this because there are such good arguments on both sides. My natural tendency is that individuals should have the choice but I would be concerned that some may feel obliged to end it all for the sake of relatives, not because that's what they want. I will be interested to hear what others think.

Minimum requirements should include the person's consent made well in advance of the situation such as with bodypart donation consent, as well as the immediate family and attending physicians.

I agree too. Incidentally DavidUK, if you find the question is long enough to hide the "Submit" button, there is a way to reveal it. Open up a reply window, then hover your mouse pointer anywhere over the question. Press and hold the left mouse button then drag the mouse downwards, highlighting the text as if you were going to copy and paste it. You will find it scrolls past the text to reveal the entire entry window and the "Submit" button. It stumped me for a while until i discovered this by accident!
