could try water filter?
I was just thinking about detergents and fat in milk (I'm a scientist!) and maybe fairy-up liquid residue was the problem, but it happens with black tea too, so I'm thinking limescale!
It may be intermittent due to the changing levels of scale in your kettle with filling andemptying with water.

Try getting some oust limescale remover (I know Wilkinsons has it because I get mine from there).
You basically boil a kettle of water, add a sachet of solution and it does its job. If still limescale then you can heat it back up but not quite to the boil and leave a bit longer. The instructions are all on the packet.
It is really cheap too - only about 1.
