how long does an E111 last?

Does an E111 last indefinitely?  I have one from when I was 16, but I think it is signed by my mum and not me.  Is this still valid or should I get a new one?  If it IS signed by me is it ok?

All E111s issued before Jan 1st this year are now invalid and have to be replaced.  These will be replaced by a plastic card from 1st Jan 06.

So, you need to apply for a new one and everyone needs their own.  Forms from PO as before.
