Me again!
I am applying for a new job and have an application form to fill in. Do I need to send a copy of my CV with the application form?

If a firm has taken the time to forumlate an application form, this implies that they do not want/need your CV. Moreover, an application form will ask questions like "what is your previous experience?", "what will you bring to our company?" and "why have you applied to work for US?" THerefore a cover letter would be surplus to requirements.
I personally would do what was asked of me. If I felt there was something extra that was REALLY important, I would add a letter. E.g., if you failed an exam but it was because you'd been ill - you may wish to explain that. But remember to make the cover letter positive over all.
Otherwise - just do what you're told - no more, no less! :-)
