After deep soul searching,I find that I wish to leave the Catholic church,Is there any formal declaration I need to make,and if so to whom ? I do not wish to be considered 'lapsed',but to return to Anglicanism.

I think you just stop going. Why is a matter between you and God. They may consider you lapsed; there's nothing you can do about that. And you can just start going to an Anglican church, though I don't know what steps , if any, they may require of you.
Best advice I can offer is to discuss this with your local Anglican priest. I'm sure he will be delighted to help and advise, especially since most of the traffic at the moment is trending in the opposite direction.
If you were originally baptised as an Anglican, I'm pretty certain that baptism will still be valid. There is no formal renunciation from Catholicism. You don't get a decree nisi, or anything like that. Just return to the church of your choice. If you weren't baptised in the Anglican church and want to receive communion there, your local vicar can arrange for your to be confirmed into the church.
