as i say i was silly and got done for DD. anyway i went to tax office today expecting to pay for my provisional and do my test all over again ( i held a full licence previously ) the guy at counter told me the dva are giving me a full licence back , i thought i had to resit a test ? i also thought i had to do a medical ? but same guy told me i do not have to do a medical , can this be true ?

what is the usual form after a 5 year ban , i was banned in 2006. will any endorsements be on my new licence ?

I think I covered your queries in my answer to your earlier question:

The endorsement remains on your licence for eleven years from the date of the offence. It is "active" for ten years - that is a second Excess Alcohol offence will result in a considerably higher sentence than if it was your first.
Was this your first offence? Anything over a three year ban is very unusual. The court should have decided if you should have to retake your test and it's up to the DVLA to decide if you need to take a medical. Mostly they don't need a medical for a first offence.
You only have to resit a test if the beak says so, there is no default of taking a test after an N year ban. Ditto the medical. The DR10 will stay for 11 years and be in effect for 10 but you've done 5 of tjose anyway. Your main problem will be insurance!
