Bizarre Diversity Forms

i ve just been asked to fill in a diversity form for my grandson who is 3 and a half ...

they want to know what his sexual orientation is ... i have replied 'hasn't made up his mind yet' as my answer!

what daft things have you been asked?

Half way through a DSS form I came across the question "Are you blind?'', I'd like to know how I was supposed to get that far through the form if I was.

Its the declined to answer options...you might as well put lesbian/gay or whatever because its unlikely a heterosexual would not want to fill that bit in...
