Present for mother inlaw

Anybody have any ideas what I can buy for a gift for my mother inlaw? She is a simple woman, never wears jewelery, wears basic clothes. She has a bad back. I just have no idea. She doesn't really have any hobbies?

Also some flowers or a plant, don't make it all about her pain. As a chronic pain suffer I know that things to help are appreciated but sometimes you can help that your pain takes over your life so something nice as well even if small would also be appreciated. Another idea is that you learn to speak some of her language, at least happy birthday, how are you today etc, that would show real effort and I am sure would make her happy.
