Mail on Sunday Rewards - I don't believe it!

I buy the Daily Mail and Mail on Sunday every weekend. Two weeks ago my Saturday edition was missing its Reward card, so I requested one through the paper by answering a couple of questions and was given a set of numbers.
Now you can only request like this twice in an eight week period, so imagine my surprise when this morning's edition also had the Reward card missing. What makes this more surprising is the fact that the magazines are packed in a cellophane wrapper! I could see remains of the glue where it should have been placed, so this suggests that something went amiss either at the printers or wholesalers.
All this is leading to me ask; does anyone have a spare set of numbers they could give me. It would be appreciated, I don't want to request a set via the paper once more, as I'd then be stuck if the same thing happened again. You can see by checking my history, I've never made this type of request before.
