I have gas central heating run from a combi boiler. Do you think I have a need for a Carbon Monoxide detector?

Yes

If anything becomes blocked,it can change the nature of the chemistry of burning with CO produced.

More than yes with a butane (butane/propane) burner as CO is easily produced from their isomers.
As the waste gas is vented to the outside I would say no, but they are very cheap and for peace of mind, I'd fit one.
I've got one on my kitchen ceiling (combi boiler in kitchen). Its piece of mind and better to be safe than sorry.
