Despite all the coverage of the report on the failings of that trust I can't find any mention of resignations.

But one person who should be "considering his position" is Sir David Nicholson (aka The Big Beast of the NHS).
He was ultimately in charge at Mid-Staffs and is now (effectively) the head of the whole NHS - not a good thought to have if you are considering being ill ...

it wasn't just a failing of the trust though sandy - appalling though it was by any clinical or management standards - it was a complete, frightening system breakdown wherever you look.
At a senior level, Cynthia Bowers, who ran nhs west midlands and presided over the winterbourne view scandal, then moved to run the care quality commission which signally failed to act on mid-staffs, was forced to resign early last year. So far,I think that's it
