Had similar a few years ago, went to a consultant at Guys with a back problem (now resolved) called by name to Consulting room, then greeted with the opening question, "Who took the Steel out of your back?" then argued when I said no-one. It was only when I stood up, turned 'round and pulled my shirt up so he could see there were no scars, that he checked the name on the notes and somewhat grudgingly agreed they weren't mine. Couldn't do anything without my notes, so a wasted 4 hour round trip.
