You won't have to replace the whole window frame (i.e. including the plastic part). The glazier will probably fit two new pieces of glass (internal and external) and new rubber seals. I'm assuming that you have sealed double glazing units, and not the secondary double glazing, where condensation will appear all the time whenever windows are open and moist air gets in through the various gaps
Both Wendy S and Ethel are correct with the exception... if the window is very old, say more than 10 years (at least here in the U.S.) the double glazed, gas filled window to be replaced now, only has about one half inch separating the two pieces of glass. The newer ones have at leat three quarters of an inch seperation and use Low-E g;ass, providing coniderably more insulating properties and UV ray protection.. This type will not be adaptable to your current frame and you may, depending on many aspects, wish to replace the entire unit...
