Statutory Declaration

I changed my name using a statutory declaration, but now I want to change it back. Can I use a statutory declaration to do that, or do I need to cancel the original one somehow?

A second statutory declaration should be valid but it might be wisest to amend the wording.

Examples: If the first declaration was worded (roughly) as "I, Fred Bloggs, hereby declare that, from this day forward, I will be known as Darth Vader" the second declaration should be along the lines of "I Darth Vader, formerly known as Fred Bloggs, hereby declare that, from this day forward I will again be known as Fred Bloggs".
