AFFIDAVIT

Name: Arif Faizan
Occupation: Civil Engineer
Employer: ABC

I, the undersigned/Arif Faizan son of Faizan Ali, Indian national, presently resident in Dubai-UAE, hereby declare on oath the following:
1. That Faizan Ali, holder of Indian passport No. ----- is my father.
2. That my father is widower as my mother Mehar Bano was expired in India on 16-10-2004.
3. That my father is staying alone in India, no body to look after him as I am only son of him staying in UAE.
4. That I am making this affidavit to get his Dependent residence visa under my sponsorship.
Further affiant saith not.
I affirm that the above and foregoing representations are true and correct to the best of my information, knowledge, and belief.

I think it should be Depandant (or Dependant's) Residency Visa.

And I think the 'Further affiant saith not' should be 'further affiant sayeth naught'

I don't know what you are trying to say here, but it's very old-fashioned language and not everyone will understand it.

Yes, it's commonly seen on affidavits, although it does sound very old-fashioned doesn't it
I've not come across it before :-)

I thought Dependant's since the noun is dependant
It's not a big point but I suggest the final comma should go in "my information, knowledge, and belief." unless you support the use of the Oxford comma or are dealing with US authorities (since the extra comma is used in the US)

esp as the US has a mad system where 10.2.2014 can be 2 Oct 2014.
