Think it is generally the response to stupid, insulting or similar statement or question. I think it means I cannot reply (perhaps without swearing) or "I'm astonished" or "I'd best say nothing". I suppose it is also used when someone asks a question to which the answer will be beyond them or shocks you like a child asking "what does f**k mean"; or someone asking how can there be a square-root of -1.
