directly to apply for any mis sold PPI's I may have.
The lady did not have the website. I have looked but there are so many coming up. She said I could apply by filling in a form that would locate any and all that I may have to show up.
Can any one help me please ? TIA

The vast majority of major institutions have been pro-active here, and have already contacted all their customers. There is no need whatsoever to use any third-party organisation to do this for you, as they will take some of the money that is owed to you. This compensation is your money, not theirs !

If you think your bank has mis-sold PPY, then the responsibility to find out is theirs, not yours or the government's.
Write to their Customer Service giving details of your account number(s) and the date(s) they were opened, and explain that you are querying PPY mis-selling and ask for a response.
Avoid any commercial sites - they will take a proportion of any money you receive for doing what you can easily do yourself for free - the process is not at all complex.
You will probably get a pro forma back to start the process.
Throughout, you must keep dated copies of your correspondence, together with all their replies. If your case is not resolved, you can involve the Ombudsman, but you will need all the details and exchanges of letters.
