The biggest reason is stress caused by poor water conditions (poor filteration, overfeeding, no water changes etc).
Like people, when fish get stressed, they're prone to many diseases such as whitespot (including ulcers which are usually a secondary infection from whitespot), fungal diseases and bacterial diseases such as finrot.

Of course, some might die from old age if they're lucky enough !!!
