Please can anyone help me I am trying to create a reference for
Palliative and end of life care guidelines for cancer and non cancer patients
third edition 2012
it has on the front NHS North of England Cancer Network (purple and white cover)It has 33 pages

I'm guessing it starts like this but unsure who the publisher is and WHERE it was published

It may not have been actually “published” ILMB. I used create such documents “in house” when I worked in the NHS. In that case, I guess the Publisher would be the North of England Cancer Network. The copy I found was on the South Tees NHS website who are these people

depends on what you are writing it for

every journal has a house style
altho Vancouver referencing used to be v popular

new england journal ( in which I have a few publications Haha ) does it like this

and used - since this is shakespeare's birthday - to have an example of quoting from the First Folio

Skakespeare, W, First Folio. London 1616.....

but I see has passed beyond ....

Drazen the new editor ( Brit, Winchester actually ) doesnt have much of a sense of humour
