A lot of people seem to worry more about Harvard referencing than they do about what they're actually writing. The truth is that hardly anyone takes much notice of references as long as:
(a) they're clearly genuine ; and
(b) they can be checked on in the unlikely event of anyone actually wanting to do so.

The following should suffice:
NHS England (2017). [i]Statistical press notice: Monthly delayed transfers of care data, November 2016[i] . [pdf] . Leeds: Government Statistical Service. Available at: https://www.england.nhs.uk/statistics/wp-content/uploads/sites/2/2016/06/November-16-DTOC-SPN.pdf [Accessed 17 Jan 2017]
Oops!

I forgot that italics don't work on AB if there's any punctuation in the relevant string. Simply put the bit between the two occurrences of [i] in my link in italics to get the result I intended.
blimey two new england journal readers n AB

their house style is here - and the referencing page is also here
and the style should be for ref 4
( in a previous edition they had a ref for a speech from Hamlet )

which should suffice

yeah and BC is right - worry about the content rather than the style - if the editor likes the text then the subeds will get onto to you about the style

oh and you are more likely to get printed if you take the view "the editor is one of the wisest men I hve ever met"
rather than the truth like " the editors choice is as usual a load of crip and quite honestly he would be better off printing me rather than one of his usual favourites "
I dont think it is for AB - chris ....
