How healthy are you?

We recently did a survey at work where all staff completed a health questionnaire to determine which areas of their lifestyle possibly needed improvement. The responses were really varied and I realised that, although some people did a lot of things right, this didn't necessarily make them happy with their bodies. This was the questionnaire. I'd be interested to know people's responses to it on here:-

Odd questions there my dear. Did not answer because I am not the healthiest person so that is that. How ever i find most people are not happy with thier body. There is something missing from this questioner anyway, what about the mental side. People miss out that they need to have to be "spiritually" happy with what ever they believe or don't. A healthy body goes with that too. The sickest people you meet are the negitive ones. Face it people just don't take all into account.
