Just after I speak I get a horrible crackling sensation in my left ear - what causes that? I notice it comes and goes and had it a few years.

Sounds as though you develop some fluid in the ear intermittently for various reasons OR that you have a scarred eardrum.

No big deal
I get that i perferated my eardrum and now its scarred thats why, the doctor will tell you with a quick look inside if its scarred or not
Wax maybe ?
I used to get a wax build up and loved having my ears syringed every couple of years. I understand there is a reluctance on the part of doctors/nurses to do this nowadays- sqad may know why
Could possibly be fluid in the inner ear.
I wouldn`t say inner ear. Possibly middle ear but more than likely the eustachian tubes. I would describe the sound as squelchy and it crackles when you speak.
Sqad appears to be busy so may I offer a dose of Ibuprofen, 600mg should do it, and monitor the situation.

As for "no big deal", I'd disagree.
Irbruprofen won`t make any diiference so there is no point in taking it in this instance (as Sqad would agree)
(Ahem), I was being facetious. :(
