He's not exactly known for having a wonderful bedside manner (because he clearly believes in directness) but his views can't be ignored. As he sees it, your only way forward is through surgery. Others might disagree though and suggest trying CBT first.
However what is abundantly clear is that you have a MEDICAL problem, which (whether it's treated by surgery, psychotherapy or anything else) clearly needs the intervention of a QUALIFIED MEDICAL PRACTITIONER. People at Weightwatchers, Slimming World or on websites such as this one simply DON'T have the necessary skills to help you.
As was written on your original thread, you CAN achieve your goal but you need to approach it ONE STEP AT A TIME. I suggest the following initial steps:
1. Read through my NHS link on your first post to understand the options that your GP might suggest (so that you're well prepared to discuss them with him/her) ;
2. As you're housebound, consider how you might be able to get to and from your doctor's surgery. (e.g. get a friend or relative to agree to drive you there).
3. Phone the surgery (or use the online booking system, if available) to make an appointment with your GP.
4. Keep the appointment.
DON'T bother setting yourself any further goals beyond those yet. Simply get those four things done and, when you get home, say to yourself "Great! I've made a start!"
If you find that something seems to be an obstacle to you achieving what you want, don't allow yourself to be beaten. Look for ways around the problem. For example, if you you can't think of any way of getting to your GP's surgery, instead of giving up, WRITE to your GP. (You can write to us, so you can write to him/her). If you can't get to a postbox, ask yourself who else could post the letter for you. (Even if there are no friends, relatives or neighbours who could help, there must be someone who calls at your house, even if it's only the Asda driver who delivers your shopping. Somebody will post the letter for you).

Can I ask how advanced your emphysema is?
If very advanced you wouldn't be a candidate for bariatric surgery, even if money were no object.
I think you need to deal with your feelings of depression and self-loathing first, which may assist your fight to lose weight in the long-run.
See your GP in the first instance and make it clear that you are ready to accept help.

The best you can take from this thread is the advice to see a doctor, in time, once you get the help you need, the feeling to want die will go, you have so much to offer and you really want to be helped. The other underlying health issues may go with that or be less chronic once you speak with medical professionals.
