Falling Asleep

I am 75, have under active thyroid, T2 diabetes, angina and erosive gastritis. I take insulin and tablets for them all. However, every afternoon when I settle down to read a book/paper, I nod off to sleep and often have an hour or more. I wake up but often drop off again as I feel so worn out. I try to get to bed before 11pm, but if I go earlier, I will lie awake. How can I feel more alert?

Nothing wrong with having a snooze during the day, I can guarantee if I pick my book up in the afternoon my eyes will close within ten minutes.
