I suspect, but cannot prove it, that the decisions to upgrade or not, are largely made by airlines a couple of days before the date of flight. They are based on the profile of the loading of the flight to date, taking into account their extensive modelling of last minute turn-up profiles of business passengers who have open tickets.
This comment is principally based on experiences with BA.
No, I wouldn't spend money on these sort of books, which if true would very quickly be disseminated widely and promptly nullify the impact of it being a secret. If it sounds too good to be true ..........

In America apparently they upgrade at the drop of a hat. So I`m told but I don`t know how true it is. Mainly, airlines upgrade when a flight is overbooked and would choose their highest tier loyalty card holders first, then work down the tiers, then pick others as they come. They won`t usually split a party, they won`t usually upgrade someone who has ordered a special meal and they only upgrade one class so someone wouldn`t manage to get from Econcomy to First Class.
Much harder these days to get an upgrade. We always travel ready to be bumped, ie spare knickers, sometimes they ask if you will be bumped off the flight onto a later flight, even if they subsequently don't need your seat you are usually given an upgrade as a reward for volunteering. We once offered to be bumped off a flight to the US in exchange for travel credit, a hotel with meals and business flights the next day. The credit paid for another trip the following year!
