I've just looked at my bank card an noticed that the chip has almost been cut out of the card and had to be physically popped back in so I'm not sure whether it's worth putting it in the machine as it could possibly get stuck or the chip might fall out and block the machine, whats the procedure do I phone the bank and tell them or will I have to go to my local branch which is not so local any more because I have moved?

You just ask for a new card which is posted to you anyway.
Phone the bank, their number should be on the card.

Ill do that tomorrow then, do you know whether you can change your banks branch to a more local one if you've moved?
you can, but it's generally pointless - you can do most things in any branch
we have lived in Hampshire for the past 20 years but our branch is still in Somerset where our first house was.
it used to be important when (for example) you had to see your bank manager to get an overdraft. Now you just apply on line!
My mates chip just fell out of his card once, he just applied online for a new one like has been mentioned. I bank with Firstdirect and have not had to use a branch for the last 15 years, they are very good
Most banks now have horrendous Call Centre phone systems where it's easier to break out of Wandworth Jail than it is to speak to somebody who can deal with your query so it's probably better to visit your nearest branch to request a new card. Most banks have totally computerised systems these days to which every branch is linked, so you don't have to deal with the branch where you originally opened your account. My account was opened miles away from where I now live and I haven't visited it for 30 years. I do all my business with the branch nearest to home.
