A one year old child would not know that you had bought him a present.  This is an expensive time for the parents, who would not be offended if the child received a Mothercare voucher.
Also, you will find that if you buy an expensive toy, the kid will probably get more fun from the box!

Buy something brightly coloured that makes lots of noise (or preferably noises) - go into mothercare and look at the age ranges marked on the labels. If you want to spend more, put some money in the child's savings account

If you are visiting this little boy, I would give him an inexpensive gift to open & put vouchers or a cheque inside his birthday card.

That way, you'll have something wrapped up in colourful paper for him to open, then he or his parents can chose something with the vouchers or cheque.

Childrens books are expensive to buy when you are parents of young children.
Also sit and ride toys or garden toys.
Ask parents what they would like.

Our wee guy can never have enough books.  He's 18 months old now and just loves them.  If anyone asks what to get him as a pressie, I suggest books, or if they don't want to risk buying duplicate books, a book token.  If the wee guy is going to be one, there are lots of great books for that age.  Charlie's favourites are ones where you lift flaps on the page to reveal hidden things, or the ones with different textures that 1yr old fingers love to explore.  Our wee guys just gets so excited every time we read them.   There's a range called Usborne touchy-feely books which are absolutely brilliant (even his dad and I dont' get fed up with them!).  I know Borders Books do them, and often have 3 for 2 offers, but you also get them in Early Learning Centres.

As the parent of an 8 month old, presents in 4 months time might include a small gift for the baby (who, has been rightly pointed out, won't notice) and a bottle of wine for the parents, delivered with a wink and a nod

Clarks vouchers - first shoes are expensive! - and a book or small toy for the baby.

A fantastic present, which has been a massive hit for the babies I have bought it for, is the ELC light and sound drum (Boots do their own version) It's 10 and all the one year olds I have bought it for, love it!

Be wary of spending 40 on a toy at this stage as they grow out of them so quick. Also big toys take up far too much room...garden toys are a good idea though. Little Tikes do a good range around the 10 mark. We bought my son a 'quad' when he was one and he still plays on it at five!
