Bath Shower Mixer Lever squirts like a fountain

Wow that was hard to define..
The bath has a shower attached (of victorian style).. the lever between the taps you move diverts the water from the bath to the shower... but this time most of the water squirted from the joint of the lever, up in the air... I could see a brass nut behind the decoration in the lever and can't find a spanner thin enough to tighten the nut... is this correct...? Or should I have baths instead!
