When I cast off 2 x 2 rib the cast-off row always fans out wider than the "body" of the rest of the rib. Anyone know how I can make it stay the same width ?

Happens often, Juniper. Depending on the garment when I cast off I usually do it using a smaller needle for the last two rows.

Are you casting off in the knit 2 purl 2 sequence, if you only cast off knitwise you will stretch the rib open

Yes, I was casting off in 2 x 2 but I've found that a smaller size needle, as suggested by gness, worked a treat.
