My daughter has a double barrelled surname on her birth certificate (ie A-B) but everywhere else, school, doctors, dentist, etc, uses only the surname B. I am now filling in some forms for her to go abroad with the school, she needs a medical form (surname B) and a EHIC Insurance Card (she hasn't got a passport). I filled the form for the EHIC Card in with surname B. Hope this will be OK. Is there any forms that need to have the actual name of her birth certificate?

No - its a group passport
I would go with the name you gave her i.e. the name on her birth certificate. Then you can't possibly go wrong.
I would have thought the EHIC will need the name A-B since that is her name. Will her name be B on the group passport?
