My company have recently decided that all empoyees should sign a form agreeing to pay for lost or stolen mobile phones (they may relent if you get a crime number).I am told that this policy has been put in place because the procurement guy didnt check that insurance was covered in the cheaper deal he signed us up to. But whatever the reason .......

As I am unable to insure someone elses property is this fair / reasonable/ usual practise?

no -it is very unusual practice. its not your fault someone c0cked up the contract and if work want you to have a phone, they should supply it, pay for it and insure it too. you must also treat the company with respect by not using it for personal calls too, tho. tell them to get stuffed, involve your union or get a job with a decent company!!!
