A relative works nights at a hospital. The hours are 5pm to 8am, a 15 hour shift , but is not allowed any breaks during that time. Is this legal?

No they have the right to a 20 min break

So are you saying that they are on the go for the whole 15 hours like the day shift?

I used to work in a hospital and at night there was a hell of a lot of sitting around...

Staff in certain occupations do NOT have the same entitlement to breaks as most other workers. (I used to run a railway station, with shifts of up to 12 hours. Breaks were extremely rare). However they should receive 'compensatory rest' at some other time. Read 'Exceptions to the regulations' in that link.
