I assume this is Driving test and you are in England

you dont HAVE to do anything - the duty to report is very restricted in English Law ( Terrorism and some money laundering spring to mind

so it depends on what you want to do
is the test - worth anything ?

I saw a student cheating in an accountancy exam thro headphones - and if he passed he could charge people for keeping their accounts - so I reported him.
afterwards.
Nothing much happened, except a notice later
"those wearing / using headphones will have their fingernails pulled out!"

you could write to the examiner at the test centre he cheated at. You may have to sign it

Not really possible to cheat with just an earpiece relaying answers.
A lot of the questions are visual, the hazard perception test and road signs, so the answers rely on sight not law knowledge.
Just failing the hazard perception part would fail the test.
How do you think you know they cheated ?

the one I had - held up a pen to the screen horizontally and went click.
what an odd things to do wivva pen I thought
then it ... clicked.

It was such and odd thing to do - hold up a pen to the screen at head height and click it

( more than once ) - and of course I never clicked my screen ever.....
