I have just found out that someone has made a fraudulent claim on my tax credits award which obviously means they have got hold of my NI number and date of birth. I am always really careful about shredding documents etc so don't know how this has happened! Have phoned tax credits helpline obviously but who else do I need to inform? Have informed my bank. Tax credits people said they would investigate and prosecute people involved as they have their bank account number so can trace them.

You will also need to inform the police and fill out a crime report. After all they have committed an offence and the tax credits people will need a crime reference number from the police as well.
it was on the news a day or two ago on either Radio 1 or galaxy saying that fraud of monetry matters needs to be reported to the bank and they will report to the police. So yyou might be able to log on to Radio 1 or Galaxy and see if you can find this news report
