Apparently the perpetrator was known to the police via 18 phone calls in the days leading up the the incident in Australia plus he was on bail in relation to sex charges and the muder / burning of his wife.

Surely the 'nutter' alarms should have gone off?

It would seem so.
Victim Katrina Dawson was killed by a police bullet. Didn't get much publicity though.

Not as much as it would have here. Maybe the Australians have a more pragmatic view of such things? I'm sure we would have made more of the ensuing police officers court case than the siege.
