It happens in many areas , to keep waiting lists down certain surgical procedures etc are carried out at private clinics/hospitals.

It is ridiculous that we have got into this state. When I moved to France, the Mairie reported my arrival and I, accordingly, got a demand from the URSSAF (financial arm of the social services) regarding my income. Given that, I later received a demand for a (means-tested) sum of money, which I duly paid and was admitted to the basic health-care scheme. It was up to me whether or not I paid the 'top-up' for full health cover - if not, then I would be liable for any excess fees over about 70%, my choice.

We do do the same to a certain extent. It's called taxes and you then have the option to pay and go private.
